Student registration

Requirements for Children:

Students must be at least the age of 5 years old and adequately potty trained to register for the SSKids Academy program.

• Florida Certification of Immunization (DH 680)/ or Immunization Exemption form
• School Entry Health Exam (DH 3040) 
• Intake/Medical History Form
• IEP 
• ABA Assessments
